Case Management- For Persons With Developmental Disabilities and Related Conditions
Who Can Receive Case Management?
You must have a diagnosis of mental retardation or a related condition, a below average IQ and deficits in adaptive behavior. These conditions must have started before your 22nd birthday.

The county will request information about you, or help you arrange for assessments to be completed by other agencies. The requested information will include:
What Can Case Management Do For Me?
Once your eligibility for services has been determined, you will be assigned a Service Coordinator. Your Service Coordinator will offer assistance in:

Identifying your needs for supports and services by assessing your basic needs, social skills and interpersonal relationships, self care skills, home living skills, cognitive abilities, health and safety concerns, communication, vocational skills, community use and legal representation.

Developing an Individual Service Plan, which identifies your service and support needs.

Locating and assisting you in accessing needed services and supports. Services can only be authorized/initiated if/when funding is available.

Coordinating the delivery of services to you.

Monitoring your services to ensure your needs are being met, and your civil and legal rights are being protected.
