Applying For Assessment

 

If you currently have Minnesota Medical Assistance, call Adult Behavioral Health Intake at 507 328-6558 to obtain an assessment.

If you do not currently have Minnesota Medical Assistance:

1. Visit the Olmsted County Campus during our walk-in times:

Mondays 1:00 PM – 5:00 PM
Wednesdays 8:00 AM – 12:00 PM
Thursdays 8:00 AM – 12:00 PM 
Fridays 1:00 PM – 5:00 PM

2. You will be asked to complete an eligibility application and invited to meet with a MNSure Navigator to apply for health insurance.  The Eligibility Application can be downloaded to complete ahead of time.


Documents needed to apply:
  • Rule 25 Eligibility Application
  • Verification of income (2 most recent proofs of income such as bank statements or pay stubs)
  • Verification of county residency

3. A social worker will make the determination of eligibility for CCDTF funding and provide further instructions on how to obtain a Rule 25 assessment.
