If you do not currently have Minnesota Medical Assistance:
1. Visit the Olmsted County Campus during our walk-in times:
Mondays 9:00 AM – 1:00 PMWednesdays 8:00 AM – 12:00 PM
Thursdays 1:00 PM – 5:00 PM

2. You will be asked to complete an eligibility application and invited to meet with a MNSure Navigator to apply for health insurance. The Eligibility Application can be downloaded to complete (94 KB) ahead of time.
3. A social worker will make the determination of eligibility for CCDTF funding and provide further instructions on how to obtain a Rule 25 assessment.
