Renew a Passport

Adult Renewal

Done only through U.S. mail

To renew a valid 10 year passport expired less than 5 years, or not yet expired.

What you must submit:

  • Completed form DS-82 - Must use black ink

  • Most recent undamaged passport

  • Check or money order payable to U.S. Department of State for the application fee according to your passport type and timeline

  • New photo - We can take this in our office

  • Any name change certified documents

Mail all of these to the correct address listed on the application according to your timing needs

If your current passport has been lost or stolen, you must fill out form DS-64 "Statement Regarding a Lost or Stolen U.S. Passport Book and/or Card" completely in black ink and return with your application.

Child under age 16

Renewal for a minor under age 16 is done at our office

What you must bring:

  • Completed form DS-11 - Must use black ink

  • Both parents must be present - If this is impossible, see step 7 on this page

  • Certified birth certificate

  • Most recent undamaged passport

  • Parents current photo ID's

  • New passport photo - we can take this in our office

  • Any name change certified documents

  • Check or money order payable to: U.S. Department of State for the application fee according to your passport type and timeline

  • Processing fee payable to Olmsted County

 Applicant age 16-17

Renewal for an applicant age 16 of 17 is done at our office

What you must bring:

  • Completed form DS-11 - must use black ink

  • We recommend one parent be present

  • Certified birth certificate

  • Most recent undamaged passport

  • Current photo ID - Or current photo ID from 1 parent

  • New passport photo - we can take this in our office 

  • Any name change certified documents for child and/or parents

  • Check or money order payable to: U.S. Department of State for the application fee according to your passport type and timeline

  • Processing fee payable to Olmsted County
